Purpose:

  • This document is specifically for CDA Home Visitor applicants undergoing the credentialing process.
  • To be completed by the #home visitor candidate’s Assessor (Mentor/Coach/Instructor) working in the candidate’s home visitor program or in the candidate’s training agency. 
  • Assessors must have been involved in the candidate’s professional life for at least 12 weeks before completing the recommendation, and have knowledge of the candidate’s professional #development and performance as a home visitor.
  • This document is used to provide a formal recommendation from the Assessor regarding the applicant's readiness for the CDA credential.
  • It serves as evidence of the candidate’s competency and the successful completion of required steps, such as the verification visit and portfolio review.

What Information Does It Ask For?

The form asks for:

  1. Assessor's Information:
    • Signature
    • Date of completion
    • Organization
    • Position
    • Address and contact information
  2. Candidate’s Information:
    • Name of the CDA applicant
    • Last four digits of the candidate’s identification number.
  3. Performance Details:
    • The Assessor is asked to describe a summary of the candidate’s performance as a home visitor working with families and their children in relation to the six CDA Competency Goals and 13 CDA Functional Areas, as outlined in the CDA Competency Standards Book for Home Visitors.
  4. Seal & Submission:
    • The document includes instructions for the Assessor to place the recommendation in a sealed envelope before returning it. This ensures the integrity of the assessment process.

How It Fits Into the Process

  • The completed and signed Assessor Recommendation is included in the CDA Professional Portfolio under Tab D.
